Urinary Tract Health

The urinary tract and associated organs are akin to the body's natural plumbing system. It functions to filter out waste and maintain a delicate balance of fluids and electrolytes. Just like any other bodily system, it requires proper care to keep running smoothly. Staying hydrated and, furthermore, being attentive to any irregularities or discomfort is crucial, as these can indicate underlying infections.

UTI Symptoms

The symptoms of UTIs can vary widely, both in the number of symptoms experienced and their severity, differing from person to person. Some people may not have obvious symptoms in spite of the presence of an infection1. In can also depend on whether a person has an upper or lower UTI. Upper UTIs affect the kidneys and the ureters (the tubes that connect the kidneys to the bladder) while lower UTIs affect the urethra and the bladder. Notably, lower UTIs are significantly more common.

Some of the most common symptoms of lower UTIs include:

  • A strong, persistent urge to urinate
  • A burning sensation when urinating
  • Frequent urination but only passing small amounts of urine
  • Signs of blood in the urine—it may appear red or bright pink
  • Cloudy or strong-smelling urine
  • Pelvic pain, in women

Upper UTIs are often accompanied by pain in the back or side (kidney pain) and/or a fever and flu-like symptoms (e.g., chills, nausea, vomiting)

UTI Prevalence

Across the globe, UTIs are estimated to afflict over 150 million people each year1. In the United States, a self-report study found that nearly 11% of adult women surveyed reported a UTI in the prior 12 months. The authors estimated that approximately 1/3 of women will have a clinically diagnosed UTI by age 243 and other sources suggest the lifetime incidence in women is approximately 50-60%4.

While UTIs are especially common in women due to a shorter urethra, men are not immune to these infections. Approximately 3 out of 25 men will in their lifetime experience the symptoms of a UTI5.

UTIs begin when bacteria—most commonly E. coli—enter the urethra and cling to the lining using adhesin proteins. The bacteria build up a glue-like biofilm6 and, if not flushed out by urination, can migrate to the bladder and wreak havoc.

Antibiotics & Recurrence

People seeking treatment for a UTI are typically prescribed antibiotics. However, reinfection is common, with about 20 to 30% of women experiencing a repeat infection within 4 to 6 months7. This cycle is problematic not only because UTIs are uncomfortable, but also because it contributes to the growing issue of antibiotic resistance8. In addition, some UTIs may be caused by a fungal infection.10

Cranberry Juice and Extract

UTIs can sometimes resolve on their own, but not in all cases, thus it is best to seek advice from a professional healthcare provider. Discomfort often prompts people to seek out natural remedies like cranberry juice. Cranberries contain proanthocyanidins which can interfere with bacteria’s ability to adhere to the walls of the urinary tract. According to the fifth update of a Cochrane meta-analysis, cranberry products may reduce the risk of symptomatic, culture‐verified UTIs in women with recurrent UTIs9. While drinking cranberry juice is considered safe and may offer some benefits, these drinks are often high in sugar, which can make them an unappealing option. As such, taking a supplement to reap the benefits of cranberries and their proanthocyanidins without the sugar is advisable.

D-Mannose

Given that bacteria, such as E. coli, adheres to the inside of the urethra and bladder using sticky biofilms, a logical approach to preventing and treating UTIs is to target this mechanism. Staying hydrated helps since regular urination can prevent bacteria from hunkering down, however, infections may still occur. Fortunately, D-mannose can help. In a randomized clinical study involving 308 women with a history of recurrent UTIs, D-mannose supplementation demonstrated efficacy in preventing future infections11. Additionally, a recent literature review concluded that D-mannose is a promising alternative to antibiotics, showing comparable effectiveness in relieving symptoms and achieving cure rates10.

D-mannose is a naturally occurring simple sugar that your body can use to cleanse the urinary tract and support a healthy bladder lining. Unlike glucose, fructose or sucrose (table sugar), D-mannose is not metabolized nor is it stored in the liver or fat cells10. Instead, it is quickly excreted in the urine about an hour after ingestion10. Accordingly, D-mannose will not impact insulin levels or interfere with healthy blood sugar regulation.
